• Consult with elderly clients and their immediate families about available

and others involved in the establishment of a plan for care and the
means to pay for it.
The Need
Old age is relatively easy to bear for two economic classes of people: the very
wealthy and the very poor.
The richest among us can afford to pay the huge cost of medical care, longterm
care, and special services from their own resources; they don’t need to ask
for permission or meet government criteria for programs.
The poorest qualify for federal and state programs that provide a safety net.
They have few if any investments and no property of any value that the government
might take.
It is the vast group in the middle who must face the most difficult of choices,
including the need to spend down their assets to pay for much of their own care and
perhaps to reach near-poverty levels before they qualify for government programs.
The role of the elder care consultant is to offer expert advice on decisions,
then to work with the client and other members of the family support team to
implement the plan. Together with the family, you may work with elder care
attorneys, accountants, and insurance agents to help implement a plan.
Among the areas you’ll need to address:
• Assessing Social Security and Medicare benefits
• Coordinating private health insurance and long-term care insurance with
Medicare
• Understanding the services available to veterans
• Reviewing Medigap insurance to fill in for areas not covered by existing
policies
• Assisting with prescription drug strategies, including Medicare prescription
cards, private programs, Veterans Administration pharmacies, and
importation of drugs from Canada or Mexico, where legal
• Knowing the eligibility requirements for Medicaid
• Protecting investments and assets by transferring them to other family
members well ahead of the point where government programs will consider
them of interest
You’ll also need to be aware of programs offered by local governments and
offices for the aging and by area religious institutions, foundations, and charities.
Challenges
This job requires a tremendous amount of research and organization, and a commitment
to keeping up-to-date on changes in laws, programs, and economics.
You’ll often be dealing with people in times of personal stress because of illness,
finances, or just the escalating concerns of old age. The elderly may have
difficulty hearing or understanding complex concepts. Every client will have different
needs.
Don’t play lawyer, doctor, or insurance agent yourself. Your job is to be an
impartial advisor to your client and an independent representative in dealings
with professionals.
Know the Territory
The federal Medicare program is essentially the same across the country. Medicaid,
which is funded jointly by state and federal governments, varies greatly from
state to state. Private Medigap insurance fills in some of the gaps in government
programs, and private health insurance and long-term-care policies can replace
or supplement government plans.
Begin by learning everything you can about the local situation. Make contact
with the county and state offices for the aging and learn about programs they
offer; read web sites and brochures produced by state and federal agencies.
Meet with area insurance agents who sell Medigap, health insurance, and
long-term-care policies to learn about available offerings. Any agent who is
especially cooperative should be at the top of your list of recommended resources
for your clients.
Check with the local bar association to find out about area lawyers who specialize
in elder law. Contact some or all of them and ask about the services they
provide; some may offer to meet with you to discuss their practice. Again, you
can include their names among your recommendations.
Visit area nursing homes and long-term-care facilities. A helpful administrator
may be able to help you learn more about the economics of long-term care.
You should be able to offer an independent assessment of the atmosphere and
conditions you find; state regulators are in charge of rating the quality of medical
care and supervision.
How to Get Started
Post your services in senior centers, community centers, and religious institutions.
Place ads in newspapers and shoppers.
Send notices of your services to area doctors and gerontologists, elder law
attorneys, and senior centers.
Offer to give a talk at senior centers and make it known that you are available
for hire for personal consultations.
Up-front Expenses
You’ll need a collection of current research and reference materials. A computer
with a connection to the Internet is also very important.
Other up-front expenses are principally for advertising and promotion.
How Much to Charge
Charge by the hour for your consultations and research. If you are asked to visit
facilities, lawyers, accountants, or other specialists on behalf of your client, you
can include a reasonable mileage charge.
